What to know first

How pregnancy changes fluid balance
Pregnancy changes circulation and encourages the body to retain fluid. As the uterus grows, pressure on veins can slow blood returning from the legs, so fluid gathers in the feet and ankles. Heat, prolonged standing and the end of the day may make symmetrical lower-leg swelling more noticeable.
Pre-eclampsia may present with sudden swelling, particularly of the face or hands, together with high blood pressure or symptoms such as a severe headache, visual disturbance or upper abdominal pain. Swelling by itself does not establish the condition, and an assessment is needed to identify the cause.
Ways to ease ordinary puffiness
Change position often, take comfortable walks if your maternity team has not restricted activity, and put your feet up when resting. Choose shoes and socks that do not pinch. Lying on your side may feel more comfortable than lying flat as pregnancy advances.
Drink normally according to thirst and avoid starting water tablets, supplements or herbal products for swelling without maternity advice. Note when the swelling began, which areas are affected and whether it changes overnight. Bring any home blood-pressure readings to your appointment, following the monitoring instructions you were given.
Symptoms that need urgent assessment
Contact your maternity unit urgently if swelling appears suddenly, worsens quickly, or comes with a severe headache, blurred vision, flashing lights, pain below the ribs, vomiting or a strong sense that something is wrong. Follow the same advice for a blood-pressure result your team has told you is outside your target range.
Get emergency help for chest pain, shortness of breath, coughing blood, collapse, or sudden pain and swelling in one leg. One-sided warmth, redness or tenderness can signal a clot and should not be watched at home.
Prepare for your appointment
Tell your clinician whether the change was gradual or sudden, if rings or shoes stopped fitting quickly, and whether one side is worse. Ask whether you need blood-pressure and urine checks, what activity is appropriate, and which symptoms should take you directly to the maternity unit.

Does swelling alone diagnose pre-eclampsia?
No. Pre-eclampsia can present with swelling, but clinicians assess blood pressure, urine, symptoms and sometimes blood tests rather than diagnosing it from swelling alone.
Can I reduce swelling by drinking less water?
Restricting fluids without clinical advice is not a suitable treatment. Drink according to thirst and ask your maternity team if another condition changes your fluid needs.
Why are my ankles puffier in the evening?
Gravity and time spent upright allow more fluid to collect in the lower legs. The growing uterus can also slow the return of blood from the legs.
