Bloody phlegm with tonsillitis: what to know

Tonsillitis causes swollen, painful tonsils and can make the throat surface delicate. Coughing, scraping off debris, or repeatedly clearing mucus may produce a small streak of blood. Strep throat is one possible cause, while viruses can cause similar symptoms. Fever, swollen glands, earache, bad breath, or white patches can help a clinician understand the pattern, but none confirms the cause alone. Blood that seems to come from the chest, rather than the throat, needs a different assessment.

Rest, drink fluids, and choose cool or soft foods if swallowing hurts. Avoid smoking, vaping, alcohol-based mouth products, and picking at the tonsils. Record the amount and appearance of blood and any fever. A GP consultation can include an examination of the throat and glands, with testing when appropriate. Antibiotic treatment helps only selected bacterial infections, so do not use someone else’s prescription or save tablets for later.

Seek emergency help if you cannot breathe, swallow saliva, or open your mouth normally, or if bleeding is more than a small streak or continues. Prompt assessment is needed for one-sided throat swelling, a muffled voice, severe dehydration, marked drowsiness, or rapidly worsening pain. Arrange review when blood returns, symptoms are not settling, or fever and weakness are significant.
