Blood-tinged discharge after sex

Friction and vaginal dryness can irritate delicate tissue, but bleeding may also come from inflammation, infection, a cervical polyp, changes on the cervix, or the womb. The appearance of the discharge cannot determine its source.

Sex can expose irritation that was not otherwise noticeable. Reduced oestrogen around or after menopause can make the vaginal lining dry, thin, and prone to small surface tears. Cervical inflammation, a sexually transmitted infection, benign growths, and changes in cervical or uterine tissue can also present with bleeding after sex.

Discharge with an unusual smell, itching, pelvic discomfort, or painful urination may accompany inflammation or infection. Bleeding after sex does not show which condition is present, and serious causes cannot be judged by the amount of blood.

Pause penetrative sex if it causes pain or further bleeding, and arrange a primary care, sexual health, or gynaecology appointment. Note whether this was the first episode, the discharge colour, the amount, contraception, menstrual or menopause status, and any new partner or infection symptoms.

The consultation may involve inspection of the vulva and vagina, a speculum examination of the cervix, and swabs where infection is possible. Cervical screening is not a diagnostic test for current bleeding, so symptoms still need assessment even if screening is up to date. If dryness is involved, the clinician can discuss suitable moisturisers, lubricants, or prescription options.

Seek urgent help if the bleeding is heavy, you feel faint, pain is severe, or you have fever and feel significantly unwell. Possible pregnancy with bleeding and one-sided pelvic pain also warrants urgent assessment.

Without emergency features, request a timely appointment if bleeding occurs after menopause, repeats after sex, happens at other times, or is accompanied by persistent abnormal discharge.

Ask whether the bleeding appears to come from the vaginal wall, cervix, or womb; whether infection testing is needed; and whether dryness could explain visible irritation. Confirm whether any imaging, tissue sampling, or follow-up examination is recommended if symptoms continue.
