Blood-streaked vomit: what it may mean and what to do

Even a small red streak should not be dismissed. Contact a clinician urgently for advice, and use emergency care if bleeding is continuing or you feel unwell.

Bright-red streaks can come from fresh bleeding in the mouth, throat, oesophagus or stomach. Forceful retching may irritate or tear tissue, while inflammation, an ulcer or another digestive condition can also bleed. Swallowed blood from a nosebleed is another possibility.

Oesophageal cancer and stomach cancer can present with vomiting blood, but this symptom alone cannot identify the cause. The direction matters: those cancers may cause this symptom; seeing a streak does not establish that cancer is present.

Dark, grainy material that resembles coffee grounds can be blood altered by stomach acid and also requires urgent assessment.

Stop eating until a clinician advises what is appropriate, especially if an urgent procedure might be needed. Do not drive yourself if you feel faint, weak or confused. Avoid taking extra aspirin or anti-inflammatory pain medicines for the bleeding unless a clinician specifically directs you.

If practical, note whether the blood was bright red, dark or coffee-ground-like, roughly how much appeared, and whether there was retching, abdominal pain, black stool or a nosebleed. Bring a list of medicines, particularly blood thinners, and describe any recent procedures or digestive diagnoses.

Seek emergency care now if there is more than a small streak, repeated vomiting of blood, coffee-ground vomit, black tar-like stool, severe abdominal or chest pain, trouble breathing, fainting, confusion, marked weakness, clammy skin or a racing heartbeat.

A small streak that stops still warrants urgent same-day clinical advice. Bleeding can recur, and appearance alone does not show how serious its source is.

Ask what source of bleeding is suspected, whether blood tests or an examination of the upper digestive tract are needed, which medicines should be paused or continued, and what change should trigger emergency care. Also ask how and when results will be explained.
