Blood spotting during sex: causes and next steps

A single light episode may follow minor surface irritation, yet bleeding that returns should not be assumed to have the same cause. A clinician can locate the source and check for conditions that need treatment.

Blood may arise from delicate vaginal tissue, the cervix or, less often, the uterus. Vaginal dryness, vigorous contact and small tears can cause surface bleeding. Cervical inflammation, a sexually transmitted infection such as chlamydia, benign growths and changes in cervical cells are other causes a clinician may consider.

Cervical cancer can present with bleeding after sex, but this symptom has many non-cancerous explanations. An examination, infection testing and confirmation that cervical screening is up to date help distinguish among them. Pregnancy testing or pelvic imaging may be appropriate depending on your cycle and other symptoms.

Arrange a primary care or gynaecology appointment if the bleeding is unexplained, happens again or occurs after menopause. Record whether it began during penetration or afterward, how much blood you saw, any vaginal discharge or pelvic pain, your last period, contraception and the possibility of pregnancy.

Pause penetrative sex if it causes pain or further bleeding. For suspected dryness, a water-based lubricant may reduce friction, but recurrent bleeding still needs evaluation rather than repeated self-treatment. Tell the clinician about a new sexual partner or possible infection exposure so suitable tests can be offered.

Seek emergency help if bleeding is heavy, you feel faint or weak, or you have severe pelvic pain. Possible pregnancy together with bleeding and one-sided pain, shoulder-tip pain or dizziness also requires immediate assessment because an ectopic pregnancy must be excluded.

Request a prompt clinic review for bleeding after menopause, persistent unusual discharge, pain during sex or repeated post-sex spotting. The need for review comes from the bleeding pattern and associated warning signs, not from an assumption that a particular diagnosis is common.

You might ask whether the blood appears to come from the vagina, cervix or uterus; whether infection tests, cervical screening or ultrasound are suitable; and when results will be available. Ask what to do if bleeding returns before follow-up.
