Bright-red blood, darker blood mixed into stool, and black or tar-like stool can arise from different parts of the digestive tract. Each pattern deserves attention rather than being assumed to come from the prostate.

Possible explanations

Common digestive causes include haemorrhoids, a small tear near the anus, bowel inflammation, or bleeding elsewhere in the gut. In someone treated for prostate cancer, radiotherapy near the rectum can irritate its lining and lead to bleeding, sometimes after treatment has finished.

A clinician may ask about stool colour, constipation, pain, bowel habit changes, abdominal symptoms, medicines, and previous pelvic treatment. Examination and testing are chosen according to the bleeding pattern and the person's overall condition.

How to respond

Arrange a medical review for new rectal bleeding, even when it seems minor. Contact the oncology team as well if it began during or after prostate radiotherapy, brachytherapy, surgery, or another cancer treatment.

Write down whether blood coats the paper, sits on the stool, is mixed through it, or makes the stool black. Record pain, constipation, diarrhoea, mucus, fatigue, and changes in bowel habit. Include blood thinners and pain medicines on your medication list, but do not alter prescribed treatment on your own.

Signs that need urgent help

Seek emergency care for heavy or continuing bleeding, black tar-like stool with weakness, fainting, confusion, severe abdominal pain, vomiting blood, clammy skin, or a racing heartbeat. These signs can indicate substantial blood loss or bleeding higher in the digestive tract.

Do not drive yourself if you feel faint or unwell. Bring your medicines and cancer-treatment details if they are readily available, without delaying the call for help.

Ask whether the bleeding pattern points to the anus, rectum, or another part of the bowel; whether an examination or bowel test is needed; and whether past pelvic treatment may be relevant. Also confirm which warning signs should bypass a routine appointment.

Pelvic radiotherapy can irritate small blood vessels in the rectal lining and cause bleeding. The oncology team should assess it because other bowel causes may produce the same sign.

Is bright-red blood different from black stool?

Bright-red blood often comes from lower in the digestive tract, while black tar-like stool may reflect bleeding higher up. Appearance is not diagnostic, and black stool with weakness or faintness needs urgent care.

Should blood-thinning medicine be stopped?

Do not stop prescribed medicine without instructions. Tell the assessing clinician what you take, because blood thinners can affect the severity and management of bleeding.
