Blood in sputum with a sore throat

Inflammation from a common cold, influenza, COVID-19, tonsillitis or strep throat can make the throat bleed slightly when mucus is cleared. Allergic rhinitis may dry and irritate the nose and throat, while acid reflux can inflame the throat without a typical cold. Glandular fever can cause marked throat pain and swollen glands. These conditions may present with a sore throat and blood-streaked mucus, but the symptom combination does not establish a diagnosis.

Look for clues such as fever, swollen neck glands, white patches on the tonsils, cough, blocked nose, hoarseness, reflux symptoms or difficulty swallowing. Blood that comes from the gums or nose may be mixed with saliva and mistaken for sputum.

Drink fluids, choose soft foods, and avoid smoking, vaping and forceful throat clearing while the tissues settle. A pharmacist or clinician can advise on suitable pain relief for you. Arrange an assessment when blood recurs, throat pain is severe, symptoms are worsening, or the cause is unclear. Antibiotics are useful only for selected bacterial infections; they do not treat every sore throat.

Seek urgent care for difficulty breathing, inability to swallow saliva, drooling, a muffled voice, rapidly increasing neck swelling, or severe one-sided throat pain. Get prompt help for more than a small amount of blood, ongoing bleeding, faintness, chest pain or severe weakness. These signs can indicate a problem needing immediate examination.
