Blood when sneezing or coughing: what to know

A dry nose, forceful sneezing, frequent nose blowing, or a cold can break small surface blood vessels. Mucus may then carry a pink or red streak into the mouth. Allergic rhinitis, including reactions to house dust mites or pets, can inflame the nasal lining and make irritation easier. Blood that is mixed with phlegm after a cough may instead be coming from the throat or lower airways. The colour and amount do not identify the cause on their own.

Sit upright and spit out blood rather than swallowing it. Avoid forceful nose blowing, repeated throat clearing, smoking, and vigorous exercise until the irritation settles. Note whether the blood appeared after a sneeze, a cough, or bleeding from the nose, and record any fever, chest discomfort, breathlessness, or medicines that affect clotting. A clinician can examine your nose, throat, and chest and decide whether testing is needed.

Seek emergency help for difficulty breathing, severe chest pain, faintness, confusion, or bleeding that is heavy or does not stop. Arrange prompt medical care if blood keeps returning, is clearly coming up with phlegm, follows a chest injury, or occurs with fever or worsening breathlessness. Children, older adults, and people taking blood-thinning medicine should seek advice sooner.
