Possible explanations for bleeding

Bleeding can come from changes in the cervix, including irritation after sex, or from an infection. It can also occur with a pregnancy loss. Sometimes an assessment does not find a clear cause and the pregnancy continues.

Ectopic pregnancy can present with vaginal bleeding along with pain that may be low on one side, shoulder-tip pain, dizziness, or fainting. These features require urgent evaluation because an ectopic pregnancy develops outside the womb and can cause internal bleeding.

Practical steps after noticing blood

Use a sanitary pad so you can observe the flow; avoid tampons. Record when the bleeding began, whether it is spotting or enough to wet a pad, its colour, and whether there are clots or tissue. Note pain, urinary symptoms, fever, nausea, dizziness, and any recent sex or examination.

Contact your maternity service, early-pregnancy service, obstetrician, or another appropriate clinician for advice. Tell them how far along the pregnancy is and whether the pregnancy location has been confirmed by ultrasound. Bring your medication list and relevant pregnancy records if you attend.

Symptoms that need immediate help

Seek emergency care for heavy bleeding, severe or worsening abdominal pain, shoulder-tip pain, marked dizziness, fainting, collapse, breathlessness, or feeling very unwell. Arrange urgent medical advice for lighter bleeding even when there is little or no pain.

If you feel faint, do not drive yourself. Use local emergency services or have someone take you for assessment. A clinician may use your history, examination, ultrasound, blood tests, or urine tests to work out the cause and next steps.
