Bloating and spotting between periods

Spotting means a small amount of vaginal bleeding outside the expected period. Bloating describes pressure, tightness, or visible swelling in the abdomen. The symptoms may share a gynaecological cause, occur at the same point in the cycle, or arise from separate reproductive and digestive factors. Their combination alone cannot identify the cause.

Breakthrough bleeding can occur with hormonal contraception, especially after a change or missed dose. Bleeding can also follow sex or occur around ovulation. Chlamydia may present with intermenstrual bleeding, discharge, pain during sex, or pelvic discomfort. Endometrial polyps and uterine fibroids may present with bleeding between periods; fibroids can also cause pressure or a sense of abdominal fullness. Cervical dysplasia and cervical or womb cancer can present with unusual bleeding, although symptoms cannot establish any of these diagnoses.

Use a calendar to mark the first day of each period and every episode of spotting. Describe the amount by whether it only marks tissue or underwear or requires period protection. Add bloating triggers, pelvic pain, bowel or urinary changes, discharge, bleeding after sex, contraception, medicines, and pregnancy possibility. A clinician may recommend a pregnancy test, testing for sexually transmitted infections, a pelvic and cervical examination, blood tests, or ultrasound. The choice depends on your symptoms, medical history, and examination findings.

Get urgent help if bleeding becomes heavy or is accompanied by sudden severe abdominal pain, one-sided pelvic pain, shoulder-tip pain, fainting, pronounced dizziness, chest discomfort, or shortness of breath. If pregnancy is possible, bleeding with abdominal pain needs prompt assessment even before the cause is known. Seek same-day advice for fever, escalating pain, or offensive discharge. Arrange prompt evaluation for bleeding after menopause or abdominal swelling that is persistent, worsening, or accompanied by reduced appetite.

Clarify whether your bleeding pattern fits a contraceptive effect and whether testing for pregnancy, anaemia, or infection is needed. Ask whether the cervix should be examined, whether cervical screening is due, and whether pelvic imaging could look for polyps or fibroids. Discuss which symptoms to monitor while waiting and when the planned follow-up should happen if spotting continues.
