Bloating and spotting after a period

The timing matters: a small amount of brown discharge immediately after the main flow can reflect older blood leaving the uterus, whereas fresh bleeding after the period seemed fully finished is bleeding outside the expected flow. Bloating may follow cycle-related fluid shifts or a bowel issue and may be unrelated to the spotting.

Hormonal changes, starting or missing hormonal contraception, irritation after sex, and pregnancy-related bleeding can alter the usual pattern. Chlamydia can present with bleeding between periods, pelvic discomfort, unusual discharge, or no other symptoms. Uterine fibroids and endometrial polyps can present with irregular or heavier bleeding and may also create pelvic fullness. Cervical or womb changes can also present with unexpected bleeding, so persistent symptoms should not be self-diagnosed.

Record when the period began and ended, whether the spotting is brown, pink, or red, how much protection it requires, and whether it follows sex. Note pelvic pain, discharge, fever, bowel changes, fullness after eating, medicines, contraception changes, and any chance of pregnancy. Take a home pregnancy test if pregnancy is possible. Arrange a routine appointment if the pattern returns, lasts beyond the end of the period, or is new for you; assessment may include a pregnancy test, infection testing, an examination, blood tests, or pelvic imaging depending on the history.

Seek urgent medical care for heavy bleeding, severe or one-sided abdominal or pelvic pain, shoulder pain, fainting, marked dizziness, breathlessness, or feeling very unwell. Bleeding with abdominal pain after a missed period or a positive pregnancy test needs prompt assessment because an ectopic pregnancy must be excluded. Fever, worsening pain, or foul-smelling discharge also warrants same-day advice. Any vaginal bleeding after menopause should be assessed promptly.

Ask whether the blood appears to come from the uterus, cervix, vagina, urinary tract, or bowel; whether pregnancy or infection testing is appropriate; and whether your contraception or medicines could affect bleeding. You can also ask what change in bleeding, pain, or abdominal swelling should trigger earlier review and whether an ultrasound or examination would clarify the cause.
