Blisters on Newborn Skin: What Parents Should Know

A blister may follow rubbing, heat, contact with an irritating product or a skin condition, but bacterial and viral infections can also present with blisters. Contact the baby's clinician the same day for advice, even if the baby otherwise seems comfortable, and avoid trying to diagnose the eruption from photographs alone.

The clinician will consider where the blisters started, how quickly new ones appeared and whether surrounding skin is red, crusted or peeling. Chickenpox presents with crops of itchy spots that evolve into fluid-filled blisters, while contact dermatitis presents where skin has encountered a trigger. These are examples of patterns, not a way to diagnose a newborn at home.

Also report poor feeding, unusual sleepiness, irritability, temperature change, breathing changes, fewer wet nappies or exposure to someone with cold sores, chickenpox or shingles. Blisters present at birth may have a different explanation from those that develop after going home.

Leave each blister intact and keep the area clean and dry. Wash your hands before and after skin care, use a clean non-stick dressing if an area is weeping, and keep the baby's nails smooth to reduce scratching. Do not apply antiseptics, steroid creams, antibiotic creams, essential oils or home remedies unless the baby's clinician directs you.

Keep anyone with an active cold sore or blistering illness away from close contact with the baby. Avoid kissing the baby, sharing towels or touching the affected skin before handwashing when a contagious cause is possible. Take clear photographs to document spread, but do not delay assessment to monitor the rash yourself.

Seek urgent medical care now if the baby has any fever, is difficult to wake, feeds poorly, has breathing difficulty, looks pale or blue, has fewer wet nappies, or develops rapidly spreading blisters, extensive peeling, pus, bleeding or marked redness. A newborn can become unwell without showing dramatic symptoms.

Blisters near the eyes or mouth, clusters of painful-looking blisters, or known contact with an active cold sore also warrant urgent clinical advice. Antiviral treatment may be considered when a clinician suspects a relevant viral infection; it should not be started, stopped or shared without medical direction.

Tell the clinician when the first blister appeared, where it began and which soaps, wipes, creams, dressings or medicines have touched the skin. Ask whether testing is needed, how to dress open areas, whether household contacts need precautions, and which feeding or temperature changes should prompt immediate reassessment.
