Blisters on the Hands, Feet and Mouth: What to Know

Small, deeply seated, very itchy blisters on the palms, sides of the fingers or soles can occur with dyshidrotic eczema. Mouth sores are not its usual presentation, so blisters or ulcers inside the mouth alongside a hand-and-foot rash may point to a different process and deserve clinical review.

Location and sensation offer useful clues. Dyshidrotic eczema presents with clusters of itchy fluid-filled bumps, followed by peeling, dryness or cracks. A rash accompanied by mouth pain, fever, sore throat or reduced appetite can fit an infectious illness instead. Friction, contact with an irritant, burns and other skin disorders can also blister these areas. Appearance alone cannot reliably establish the cause.

Note whether the spots itch, hurt or spread; whether the fluid is clear or cloudy; and whether anyone nearby has similar symptoms. These details help a clinician separate eczema from infection or injury.

Do not deliberately burst the blisters. Wash the skin gently, pat it dry and cover open areas with a clean, non-stick dressing. Avoid fragranced products, harsh cleansers and activities that repeatedly wet or rub the affected skin. Cool drinks and soft foods may be easier if the mouth is sore; maintaining fluid intake matters more than eating a full meal temporarily.

Do not share towels, cups or utensils while an infection is possible, and wash hands after touching the rash. A clinician may recommend a topical corticosteroid when dyshidrotic eczema is confirmed, but steroid creams should not be applied to an unexplained blistering rash or inside the mouth without specific instructions.

Arrange prompt assessment if the rash is widespread, rapidly worsening, very painful, recurring, or accompanied by fever. Seek urgent care for difficulty breathing or swallowing, swelling of the lips or tongue, marked drowsiness, confusion, or signs of dehydration such as very little urine, a very dry mouth or inability to keep fluids down.

Increasing warmth, redness, swelling, pus, red streaks or worsening tenderness around a blister can signal a secondary skin infection. People who are pregnant, have weakened immune defences or are caring for a very young infant should obtain individual advice when exposed to a blistering illness.

Ask which diagnosis fits the distribution, whether the rash could spread to other people, and how to care for open areas. Also clarify which products are safe for the skin, whether mouth pain needs separate treatment, and what change should trigger reassessment. Bring a list of recent medicines, new skin products and possible irritant exposures.
