Blepharoplasty with Fat Transfer

Fat transfer and fat grafting usually describe the same general process: fat is taken from a donor area, prepared and placed in small parcels. During blepharoplasty, the surgeon may also remove skin, preserve or reposition existing eyelid fat, and support the lid when needed. Added fat is intended to smooth selected depressions rather than simply make the eyelid fuller.

A hollow shadow does not always come from lost volume. Prominent eye bags, cheek position, thin skin, pigmentation and lighting can create a similar appearance, and adding fat will not correct each cause. Possible outcomes include persistent swelling, bruising, lumps, asymmetry, contour irregularity or volume that settles differently on each side. Rare vascular blockage around the eye can threaten skin or vision.

Ask for an examination that includes the brow, upper and lower lids, eye closure, lid support and the lid-to-cheek transition. Share any history of dry eyes, facial injections, previous eyelid surgery, easy bleeding, unusual scars or major weight change. Photographs from before earlier treatments can help distinguish longstanding anatomy from a new contour change.

Confirm whether the plan uses harvested fat, repositions existing eyelid fat, or does both, because the terms can sound similar. Follow instructions on medicines, fasting, transport and cleansing. During recovery, keep incisions clean, use only prescribed eye products, avoid pressure over transferred fat and postpone strenuous activity until the surgeon says healing is adequate.

Treat sudden visual change, severe pain in or behind the eye, a curtain-like shadow, new weakness or speech difficulty as an emergency. Immediate assessment is also appropriate for rapidly enlarging eyelid swelling or bleeding that continues despite the team's instructions. Notify the surgeon without delay if redness spreads, pain worsens, pus appears, fever develops or the eyelid no longer closes over the eye.

Ask what is causing each visible hollow or bulge, whether existing lid fat can be repositioned, and why harvested fat would add value. Have the surgeon mark the intended donor and recipient areas, explain how transferred volume may change as swelling settles, and outline options for a lump, asymmetry or under-correction. Confirm whom to contact immediately if vision changes after discharge.
