Blepharoplasty with Fat Grafting

Blepharoplasty can remove selected loose skin and reshape or reposition eyelid fat. Fat grafting uses fat collected from another body area, processes it and places small amounts around chosen contours. The combination may soften a hollow transition between the lower eyelid and cheek or restore volume near the upper lid while preserving normal eyelid movement.

Transferred fat behaves as living tissue, but not every placed cell remains. Swelling can temporarily hide the contour, and the settled result may be uneven or require further correction. Other risks include bruising, infection, lumps, overfilling, underfilling and donor-site irregularity. Placement around the eye demands detailed anatomical knowledge because fat entering a blood vessel can cause severe harm, including visual injury.

Choose a clinician trained in eyelid surgery and facial fat placement. Assessment should separate true hollowness from eye bags, loose skin, cheek descent, lid laxity and a low brow. Review dry-eye symptoms, previous fillers or operations, bleeding tendency, smoking, medicines and whether enough suitable donor fat is available.

Before treatment, agree on the donor site and where fat will be placed. Arrange help for the early recovery period and follow instructions for cold compresses, head elevation, wound care and activity. Do not press or massage grafted areas unless directed, because placement and healing are deliberate. Attend scheduled checks even if swelling seems symmetrical.

Sudden loss or dimming of vision, a new visual-field defect, severe eye pain, weakness, facial drooping or trouble speaking requires emergency care. Rapidly expanding swelling, uncontrolled bleeding or difficulty breathing also needs immediate attention. Contact the surgeon promptly for worsening one-sided pain, increasing redness, discharge, fever, a cold pale patch of skin or inability to close the eye.

Ask which features will be treated by blepharoplasty and which by grafting, how conservative volume placement will be, where fat will be harvested and how asymmetry would be managed. Discuss the surgeon's technique around blood vessels, the emergency plan for a visual symptom, likely donor-site changes and whether existing filler should be dissolved or left alone.
