Blepharoplasty Scar Revision

Early eyelid scars may look pink, firm or slightly raised before they soften. Revision is considered when a scar remains conspicuous, pulls on the eyelid, causes discomfort, traps skin, or sits outside a natural crease. The plan depends on whether the problem is excess scar tissue, wound separation, pigment change, an uneven incision line or altered lid position.

Treatment is not always another operation. Depending on the examination, care may include scar massage after the wound has closed, silicone-based products, a targeted injection, light or energy treatment, or surgical removal and careful re-closure. A revised scar still heals as a new scar, so improvement rather than erasure is the realistic goal.

Bring the operation details, photographs showing how the scar evolved and a list of products already applied. Describe dryness, tearing, double vision, lid tightness and whether the eye closes fully during sleep. The surgeon may wait for ongoing scar remodelling unless the scar is distorting the eyelid or threatening the eye surface.

Use only scar products approved for the eyelid and keep them out of the eye. Protect the line from strong sun, avoid rubbing, and do not restart contact lenses or eye makeup until cleared. Smoking and unreported medicines that alter bleeding or healing can affect the revision plan.

Arrange urgent assessment for reduced vision, severe or escalating eye pain, sudden marked swelling, persistent bleeding, or inability to shut the eyelid. A scar that pulls the lid away from the eye can expose the cornea and should not wait for a routine cosmetic review. Spreading redness, warmth, discharge, fever or a wound that opens also warrants prompt contact with the surgical team.

Ask which part of the scar is still changing, whether eyelid position or eye closure is affected, and what outcome each proposed method can reasonably deliver. Clarify where any new incision would lie, how recurrence or pigment change would be handled, what aftercare is safe beside the eye and whether another specialist should assess the ocular surface.
