Blepharoplasty for under-eye bags

A surgeon evaluates the bag itself, lid support and the transition into the cheek. Surgery may remove or reposition fat and address skin laxity, depending on the anatomy and eye-surface health.

A persistent bulge may come from fat projecting forward as supporting tissues change. Loose skin can add creasing, while a tear trough hollow below the bulge casts a shadow that makes the bag look more prominent.

Fluid-related puffiness often varies during the day and may respond poorly to surgery aimed at fat. Dark colour can arise from pigment, visible vessels or shadowing, so removing tissue does not guarantee a brighter under-eye area.

Note whether puffiness changes with sleep, allergies or irritation, and bring a full medicine and supplement list. Mention dry eyes, previous laser vision correction, eyelid procedures and any difficulty closing your eyes.

The examination may cover lower-lid tone, eye prominence, fat distribution, skin quality and cheek support. The surgeon can then explain the incision route, whether fat is removed or repositioned and how the lid will be supported. Plasma pen treatment does not reproduce surgical fat repositioning.

New one-sided swelling with pain, redness, fever, discharge, double vision or reduced vision needs prompt medical assessment rather than cosmetic treatment. Breathing difficulty or swelling of the lips or tongue requires emergency help.

Following lower eyelid surgery, contact the surgical team urgently for deteriorating vision, intense eye pain, rapidly increasing swelling, ongoing bleeding, a lid pulling away from the eye or inability to close it.

Ask whether fat, fluid, skin laxity or a hollow creates most of the appearance; how lower-lid support will be protected; what change is realistic for dark circles; and how postoperative eye irritation will be managed.
