Is bleeding normal in early pregnancy?

The colour or small amount of blood cannot confirm that the pregnancy is healthy or identify the cause. A clinician may use your symptoms, examination, ultrasound or pregnancy-hormone blood tests to work out what is happening.

Spotting may come from the cervix, which becomes more prone to bleeding during pregnancy, particularly after sex or an examination. Bleeding can also occur with an infection, an early pregnancy loss or a pregnancy developing outside the uterus. An ectopic pregnancy may present with vaginal bleeding, often alongside pelvic or abdominal pain, although symptoms vary.

Bleeding ranges from a few pink, red or brown marks to a flow with clots. Its appearance alone does not reliably separate a minor cause from one needing treatment, so the surrounding symptoms and a medical assessment matter.

Contact your obstetrician, midwife, early-pregnancy service or clinic and explain that you are pregnant and bleeding. Note when it began, whether it is increasing, the colour, whether there are clots or tissue, and any pain, fever, dizziness or faintness. Use a sanitary pad so you can monitor the flow; avoid inserting a tampon while you are bleeding.

Tell the clinician the date of your last menstrual period, any previous ectopic pregnancy or pregnancy loss, and any medicines you take. Follow the service's advice about examination, scans and blood tests rather than relying on a home pregnancy test to explain the bleeding.

Go to the nearest emergency department or call local emergency services now if bleeding is heavy, you are soaking pads quickly, or you have severe or one-sided abdominal or pelvic pain, shoulder-tip pain, marked dizziness, fainting, weakness, breathing difficulty, confusion or pale, clammy skin. These signs can accompany significant blood loss or internal bleeding and should not wait for a routine appointment.

Seek prompt same-day medical advice if the bleeding is getting heavier, pain is worsening, or you develop fever, chills or unpleasant-smelling discharge. If you cannot reach your usual maternity team, use an urgent-care or emergency service.

You could ask what causes fit your symptoms, whether an ultrasound or blood test is appropriate now, when any test should be repeated, and exactly which changes should take you to emergency care. Also ask whom to contact if bleeding continues outside clinic hours and whether you need to adjust any prescribed medicine or activity while waiting for assessment.
