Bleeding near the placenta in early pregnancy

The blood may be noticed vaginally or appear as a collection beside the pregnancy on an ultrasound. Its significance depends on the scan findings, the amount and pattern of bleeding, pain, and how the pregnancy is developing.

A scan may describe a small collection of blood between the pregnancy tissues and the wall of the uterus, sometimes called a subchorionic haematoma. Blood can later pass through the vagina, look brown as older blood leaves, or remain visible only on imaging.

Bleeding can also arise from the cervix, which has a richer blood supply during pregnancy. Miscarriage can present with bleeding and cramping. Ectopic pregnancy can present with vaginal bleeding together with pelvic or one-sided abdominal pain, so the location of the pregnancy must be confirmed when it is not already clear.

Contact your maternity team or gynaecology service and describe the colour, flow, clots, pain, dizziness, and when the bleeding began. Use a sanitary pad rather than a tampon so you can monitor the flow. Avoid changing prescribed medicines unless the clinician managing your pregnancy advises it.

Assessment may include questions about your dates and previous pregnancies, an examination, an ultrasound, and blood tests. A repeat scan may be suggested when an early scan cannot yet show enough detail. Follow the activity and follow-up advice given for your individual scan findings.

Go to an emergency department now for heavy or rapidly increasing bleeding, severe abdominal or pelvic pain, shoulder-tip pain, fainting, marked dizziness, weakness, breathlessness, or feeling acutely unwell. These signs need prompt assessment regardless of what an earlier scan showed.

Seek same-day advice for new bleeding in pregnancy even when it is light, especially if it recurs or comes with cramps. If your blood group is Rh-negative, mention this when you call because the care team may need to consider an injection based on the circumstances and stage of pregnancy.

Ask where the blood collection is located, whether the pregnancy is confirmed inside the uterus, what the scan shows about development, and when follow-up is needed. You can also ask which changes in bleeding or pain should take you directly to emergency care.
