Bleeding at four weeks pregnant: what to do

Light spotting does not by itself show whether a pregnancy is progressing normally. The amount, trend, pain, examination findings and appropriately timed tests provide more information than blood colour alone.

Bleeding can come from the cervix, the vagina or within the uterus. Possible explanations include cervical sensitivity, a small collection of blood near the pregnancy, early pregnancy loss or a pregnancy developing outside the uterus. Ectopic pregnancy can present with vaginal bleeding and pelvic or abdominal pain, sometimes before a pregnancy location can be confirmed on a scan.

At this early stage, dates based on the last period may not precisely match development. A single test or very early scan may therefore be inconclusive and may need follow-up arranged by a clinician.

Contact your maternity service, obstetric clinician or a medical clinic and describe the bleeding. Note when it began, whether it is increasing, any clots or tissue, and the location and severity of pain. Use a pad to observe the flow and avoid tampons while bleeding.

Tell the clinician about previous ectopic pregnancy, tubal surgery, fertility treatment or an intrauterine device if any applies. Assessment may involve an examination, ultrasound and repeat pregnancy-hormone blood tests. Follow the service's timing advice because results must be interpreted together.

Go to emergency care now for severe or worsening abdominal or pelvic pain, pain mainly on one side, shoulder-tip pain, fainting, marked dizziness, confusion, unusual paleness or difficulty breathing. These can accompany internal bleeding and require immediate assessment.

Urgent assessment is also needed when vaginal bleeding is heavy or rapidly increasing, or when you feel significantly unwell. Do not drive yourself if you feel faint; call emergency services or have someone take you.

Ask whether the pregnancy location can be identified yet, whether blood tests or another scan are needed, and exactly when follow-up should occur. Confirm which pain or bleeding changes mean you should return immediately and whom to contact outside clinic hours.
