Bleeding and clots in the first trimester

Light spotting can occur in an ongoing pregnancy, while heavier bleeding or passed tissue may accompany pregnancy loss. An ectopic pregnancy may also cause vaginal bleeding, sometimes with pelvic pain, and must be excluded when symptoms suggest it.

Blood can collect before leaving the vagina and form clots, so clotting does not reveal whether a pregnancy is continuing. Possible explanations include bleeding from the cervix, a small collection of blood near the pregnancy, pregnancy loss, or an ectopic pregnancy. Only an assessment can distinguish among them.

A clinician may ask about the amount and colour of blood, pain, dizziness, pregnancy dates, and whether tissue was passed. Depending on the situation, evaluation may include an examination, ultrasound, blood tests, and repeat testing to see how findings change.

Contact your maternity team, obstetrician, or an urgent care service promptly for advice, even if the bleeding settles. Use a sanitary pad rather than a tampon so you can monitor the flow. Note when it began, whether it is increasing, the size of clots, and any cramps or one-sided pain.

If you pass material that may be pregnancy tissue, ask the care team whether they want it brought for assessment. Tell them your blood group if known, your medicines, previous ectopic pregnancy or miscarriage, and any fertility treatment. Do not stop prescribed medicine unless a clinician directs you.

Go to an emergency department or call local emergency services for severe or worsening abdominal pain, pain focused on one side, shoulder-tip pain, fainting, marked dizziness, weakness, difficulty breathing, confusion, or bleeding that rapidly soaks through pads. These signs can accompany significant blood loss or internal bleeding.

Urgent assessment is also appropriate for fever, chills, foul-smelling discharge, or feeling acutely unwell. Do not drive yourself if you feel faint or weak; arrange emergency transport or have another adult take you.

Ask whether the pregnancy location and development can be seen yet, whether blood tests or another ultrasound are needed, and which changes should bring you back immediately. You can also ask whether your blood group affects treatment, what pain relief is suitable, and whom to contact if bleeding returns outside clinic hours.
