Bleeding Between Periods After Tubal Ligation

Short-term bleeding soon after the procedure may relate to surgery or uterine instruments used during it. Bleeding that starts later, keeps returning, or comes with pain may have a separate gynaecological cause.

Tubal ligation blocks or interrupts the fallopian tubes; it does not stop the ovaries from cycling or the uterine lining from shedding. A change noticed after surgery may instead coincide with stopping hormonal contraception, which can alter a previously medication-controlled bleeding pattern.

Other explanations include infection, cervical irritation, endometrial polyps, or uterine fibroids. Chlamydia can present with spotting, pelvic pain, discharge, or urinary discomfort. Cervical disease and womb cancer may present with unexpected vaginal bleeding, but bleeding alone does not identify its cause.

If the operation was recent, follow the discharge instructions and contact the surgical team when bleeding exceeds what they told you to expect, the wound becomes red or drains fluid, or pain is increasing. Record cycle dates, spotting, flow, pain, discharge, sex, and medicine or contraception changes.

Pregnancy can still occur after tubal ligation. Take a pregnancy test for a late period, pregnancy symptoms, or unexplained spotting. Arrange an appointment for recurring bleeding, bleeding after sex, or a sustained change in your cycle; assessment may include pregnancy and infection tests, examination, or imaging.

Seek emergency care for a positive pregnancy test with bleeding or pelvic pain, severe one-sided abdominal pain, shoulder-tip pain, fainting, or marked dizziness. Pregnancy after tubal ligation requires prompt evaluation because it may be ectopic.

Urgent assessment is also appropriate for heavy bleeding, worsening pain, fever, vomiting, abdominal swelling, or feeling very unwell after recent surgery. Any vaginal bleeding after menopause should be evaluated promptly.

Tell the clinician when the procedure occurred, which technique was used if known, and whether hormonal contraception changed afterward. Ask whether the timing suggests postoperative bleeding, whether pregnancy and infection testing are needed, and whether an examination or ultrasound would help locate the source.
