Bleeding after sex with lower abdominal pain

Arrange a prompt appointment even if the bleeding is light. Seek urgent help sooner if the pain is severe or one-sided, you feel faint, or pregnancy is possible.

Bleeding can come from friction, vaginal dryness, inflammation, or sensitive tissue on the cervix. Pain alongside it broadens the possibilities to pelvic infection and conditions involving the uterus or ovaries. Pelvic inflammatory disease can present with pelvic pain, unusual discharge, pain during sex, or bleeding after sex. Endometriosis, adenomyosis, ovarian cysts, and fibroids may also cause pelvic pain or abnormal bleeding, although the timing after sex does not identify a cause by itself.

If pregnancy is possible, an ectopic pregnancy must be considered when bleeding occurs with lower abdominal pain, particularly if the pain is one-sided. An examination and testing are needed to distinguish these causes.

Pause penetrative sex if it worsens pain or bleeding. Note when the symptoms began, where the pain is felt, how much blood you see, your last menstrual period, contraception, discharge, fever, and any chance of pregnancy. Take a home pregnancy test if pregnancy is possible, but do not delay urgent care for a negative result when severe symptoms are present.

A clinician may examine the abdomen, vagina, and cervix and may recommend pregnancy, infection, or imaging tests based on the findings. Treatment depends on the cause rather than the bleeding alone.

Go to an emergency department for severe or rapidly worsening abdominal pain, fainting, marked dizziness, shoulder-tip pain, heavy bleeding, difficulty breathing, or pale and clammy skin. These signs are especially urgent when pregnancy is possible or a pregnancy test is positive.

Seek same-day medical advice for fever, vomiting, foul-smelling discharge, increasing pelvic tenderness, or bleeding that continues. Recurrent bleeding after sex also warrants a scheduled examination even when the pain settles.

Ask whether you need a pregnancy test, infection testing, cervical screening, or a pelvic ultrasound. You can also ask what may safely ease the pain, whether to avoid sex until results return, and which changes should send you to urgent care.
