Black watery stool meaning: when to act

Melaena is often black, sticky, and strong-smelling because blood has been changed by digestion. Gastritis, a peptic ulcer, and Helicobacter pylori infection are examples of upper digestive problems that can be associated with this finding. Colour is not a diagnosis: watery stool after a dark food, iron tablet, or bismuth remedy may have a different explanation, especially when you otherwise feel well.

Check the timing of medicines, supplements, and foods, and note whether the stool is sticky, foul-smelling, or repeatedly black. Keep drinking small amounts if you are losing fluid and can swallow safely. Arrange prompt medical advice for a new or unexplained change; do not rely on a home colour check to rule out bleeding or stop prescribed treatment on your own.

Go to emergency care if you feel faint, confused, breathless, unusually weak, or clammy, or if black stool comes with severe abdominal pain, vomiting blood, or coffee-ground vomit. Reduced urination and inability to keep fluids down also need urgent attention because watery stool can cause dehydration alongside another illness.

Ask whether you need tests for bleeding, an ulcer, gastritis, or Helicobacter pylori infection. Tell the clinician about the number of episodes, recent antibiotics or travel, stomach symptoms, previous digestive disease, and all prescription and non-prescription products. Ask what change should prompt immediate return.
