What should you know about black scales on skin?

Psoriasis, seborrheic dermatitis, ringworm, and tinea versicolor can present with scaling, although each has a different typical pattern. On deeper skin tones, inflammation beneath the scale may look dark brown, grey, or purple.

Thick, sharply outlined plaques favour a different process from fine flakes in oily scalp or facial areas. An enlarging patch with a scaly border can occur with ringworm, while tinea versicolor commonly presents with fine scale and altered colour on the trunk or shoulders.

Black crust after scratching may contain dried blood. A tender black surface, bad smell, drainage, or skin that is cold or numb raises concern for tissue injury rather than ordinary scale and should not be scrubbed away.

Soften dry scale with a plain fragrance-free moisturiser and wash with lukewarm water and a gentle cleanser. Pat rather than rub. Avoid picking, abrasive gloves, strong peels, and sharing towels or combs when a fungal infection is possible.

Note the body site, itch or pain, border shape, moisture or oiliness, hair loss, and whether the scale returns after gentle washing. Because steroids and antifungals treat different problems, seek guidance before applying a medicated combination cream to an uncertain rash.

Book a review when dark scaling persists, spreads, involves the scalp with hair loss, affects the nails, or does not respond to gentle care. Examination is also appropriate for recurrent patches or when a household member has a similar itchy rash.

Seek prompt care for fever, rapidly spreading redness, severe pain, swelling, pus, blisters, an open wound, or black tissue that feels numb or cold. These findings can signal infection, injury, or impaired blood flow rather than routine flaking.
