Birth control and hirsutism

The choice depends on pregnancy plans, other symptoms, medicines, and health factors that affect whether oestrogen-containing contraception is suitable. Improvement is gradual because existing hairs must complete their growth cycle.

Hirsutism describes coarse, darker hair growing in an androgen-sensitive pattern, such as on the face, chest, abdomen, or back. It can occur with polycystic ovary syndrome, certain medicines, or other hormonal changes, and sometimes no single cause is found.

Combined hormonal contraception can reduce ovarian androgen production and increase proteins that bind circulating androgens. Formulations differ, so a pill that suits one person may not suit another. Progestin-only contraception is not generally used as a treatment for hirsutism.

Arrange an assessment before starting or changing contraception. Bring a medicine list and mention irregular periods, acne, scalp thinning, weight changes, pregnancy plans, migraine with aura, smoking, previous blood clots, or liver disease because these details can change the safest option.

Track new growth with occasional photographs rather than checking daily. Shaving, waxing, threading, laser hair removal, or electrolysis can address existing hair while hormonal treatment targets new growth. Spironolactone may be considered in selected patients and requires reliable pregnancy prevention and clinical monitoring.

Book a prompt appointment if coarse hair appears rapidly, or comes with a deeper voice, increased muscle bulk, severe acne, new scalp hair loss, or marked menstrual changes. These features warrant assessment for a stronger androgen effect or another underlying condition.

Seek urgent care after starting hormonal contraception for sudden chest pain, breathlessness, coughing blood, one-sided leg swelling, fainting, a new severe headache, weakness, or vision disturbance.

Ask whether your hair-growth pattern suggests a hormonal evaluation, which contraceptive formulations fit your medical history, how progress will be assessed, and whether a hair-removal method or anti-androgen medicine could be combined with contraception.
