Biological treatment for ulcerative colitis: side effects

Some people notice redness, swelling, itching or soreness where an injection was given. During or soon after an infusion, a person may develop flushing, headache, dizziness, rash or breathing symptoms. The care team can observe you and explain which changes are expected for your medicine.

Because biological medicines modify parts of the immune response, infections may need attention sooner. Fever, chills, a persistent cough, painful urination, worsening skin redness or unusual weakness should be reported. Diarrhoea, abdominal pain and blood can also reflect active ulcerative colitis rather than a side effect, so assessment matters.

Follow the medicine instructions and keep a record of when a symptom starts, how long it lasts and whether it follows an injection or infusion. Tell the prescribing team about new symptoms before taking the next dose. Do not restart, skip or stop treatment based only on a symptom without advice.

Mention all medicines, supplements, allergies, recent infections and vaccinations to your clinician. Ask what to do if you are unwell on treatment day and whether you need a review before the next dose. A GP consultation can help assess a new problem and coordinate with your specialist.

During or after an infusion, get immediate help for trouble breathing, swelling of the lips or tongue, widespread hives, severe dizziness or fainting. These can signal a serious allergic reaction. Follow the treatment centre's emergency instructions and do not wait for symptoms to settle.

Seek urgent care for severe or worsening abdominal pain, a swollen abdomen, repeated vomiting, heavy rectal bleeding, confusion or fainting. Contact your treatment team promptly for fever, persistent cough, worsening diarrhoea or other signs of infection. Symptoms alone cannot show whether the cause is treatment or ulcerative colitis.
