Biological therapy for ulcerative colitis: what to know

Ulcerative colitis causes inflammation in the lining of the large bowel. Biological medicines act on selected parts of the immune response involved in that inflammation. They may be offered when symptoms remain active despite other medicines, when disease is difficult to control, or when a specialist considers them suitable for long-term management.

Treatment may be given through an injection under the skin or an infusion into a vein. The schedule depends on the medicine. Before starting, your specialist may review infections, vaccinations, previous treatments and other health conditions. Monitoring helps the team assess symptom control and watch for problems during treatment.

Keep appointments and follow the prescribed schedule, even when bowel symptoms have settled. Record changes such as bowel frequency, blood, urgency, abdominal pain or fatigue so your team can judge how the medicine is working. Do not stop treatment or delay a dose without speaking with the prescribing team.

Tell the team about fever, a new infection, recent antibiotics, planned procedures, pregnancy or medicines that affect the immune system. Ask which vaccines are appropriate before treatment and what to do if you miss an injection or infusion. A GP consultation can help coordinate care when symptoms change.

Contact your treatment team promptly if you develop a fever, persistent cough, painful skin changes, unusual tiredness, or symptoms of an infection after starting therapy. Seek urgent medical help for severe or worsening abdominal pain, a swollen abdomen, repeated vomiting, heavy rectal bleeding, fainting or confusion.

A flare may include more diarrhoea, urgency or blood, but these symptoms can have several causes. Do not assume they are a medication effect or a flare without assessment. If you have ongoing diarrhoea or abdominal pain, arrange clinical advice so treatment can be reviewed.

Ask which biological medicine is being considered, how it will be given, when improvement should be assessed, and which symptoms require a call.
