Bile acid levels in obstetric cholestasis

Bile acids are substances made by the liver to help digest fats. Obstetric cholestasis, also called intrahepatic cholestasis of pregnancy, reduces normal bile flow and allows bile acids to build up in the blood. Itching without a rash, particularly on the palms and soles, is a characteristic presentation, but symptoms and test results do not always begin together.

A laboratory interprets the result using its reporting range and the maternity service's protocol. One result is only a snapshot: levels can rise or fluctuate as pregnancy continues. Liver enzyme tests add information about liver irritation but do not replace a bile acid measurement for assessing this condition.

Contact your maternity team if you develop persistent itching, even if an earlier blood test was within the laboratory range. Repeat bile acid and liver blood tests may be arranged when itching continues because biochemical changes can appear later. Take medicines exactly as prescribed and attend the planned maternal and fetal checks.

The team considers the highest and most recent bile acid results alongside gestational stage, symptoms and other pregnancy factors. This discussion may include timing of birth and whether induction of labour is appropriate. Do not change medication or bring forward delivery plans based on a result without speaking to the maternity clinician responsible for your care.

Call your maternity unit promptly if your baby's movements are reduced or different from their usual pattern; do not wait for the next appointment. Urgent assessment is also needed for vaginal bleeding, severe abdominal pain, regular painful contractions, leaking fluid, severe headache, visual disturbance, yellowing of the eyes or skin, or feeling acutely unwell.

Ask which bile acid result the team is using for decisions, when blood tests will be repeated and how you will receive new results. Clarify what treatment is intended to do, how fetal wellbeing will be assessed, what changes in movement require immediate contact, and how the timing and method of birth will be reviewed.
