Why both areas may hurt
The big toe carries load during walking, while the thumb handles pinch and grip, so each can be irritated by its own demands. Pain beginning after new footwear and hand-intensive work may therefore have unrelated explanations. When several joints become painful or swollen around the same time, a clinician also considers inflammatory arthritis, crystal-related inflammation, infection and other medical causes. De Quervain's tenosynovitis commonly presents with pain along the thumb side of the wrist, but it would not account for big toe pain.
Track the pattern and reduce strain
Note which area started first, whether symptoms appeared suddenly, and whether there is warmth, redness, swelling or prolonged stiffness after rest. Check footwear for pressure over the toe and ease back from painful pushing-off, gripping or pinching. Choose roomy shoes and adapt hand tasks without stopping all comfortable movement. Do not start leftover prescription medicine or attempt to drain a swollen joint. An examination can determine whether blood tests, fluid sampling or imaging would add useful information.
When the pattern is urgent
A joint that becomes intensely painful, hot, red and swollen over a short period needs prompt assessment, particularly when fever, chills, a wound or feeling unwell is present. Seek urgent care after trauma if you cannot bear weight, cannot use the hand, notice deformity or lose sensation. Otherwise, arrange an appointment if more joints become involved, stiffness persists after rest, episodes recur or everyday walking and gripping remain limited.

Does pain in both places prove arthritis?
