The key point

Pain may be provoked by bending the elbow against resistance or turning the palm upward, such as when using a screwdriver or carrying a load palm-up. Forearm pain also has other sources, so symptoms away from the front elbow need a broader assessment.

Clues from location and movement

The distal biceps tendon crosses the front of the elbow and attaches to a forearm bone. Repeated pulling or a sudden heavy load can irritate or injure it. Outer-elbow pain with gripping may accompany tennis elbow, whereas inner-elbow pain may accompany golfer's elbow. Tingling, burning or altered hand sensation points toward possible nerve involvement rather than an isolated tendon problem.

Because several tissues work during the same tasks, a clinician may test resisted elbow bending, palm rotation, grip, neck movement and sensation before identifying the likely pain source.

Reducing irritation without losing movement

Temporarily reduce heavy palm-up curls, forceful pulling and repetitive twisting that reproduce the pain. Use a neutral grip when practical and divide awkward loads. Keep the elbow moving comfortably rather than holding it completely still.

Once ordinary tasks settle, resistance can be rebuilt in a controlled way. Begin below the load that triggers a flare, monitor the response later that day, and progress gradually. Persistent symptoms benefit from assessment because a partial tendon injury and routine overload may require different loading plans.

Warning signs after an arm injury

Arrange prompt evaluation if a sudden load caused a pop, bruising around the front of the elbow, a changed biceps contour, or a clear loss of strength when bending the elbow or turning the palm upward. Seek urgent care for a cold or discoloured hand, rapidly worsening swelling, severe unremitting pain, or new major numbness or weakness.

An appointment is also sensible when pain does not improve with load modification or repeatedly blocks work, training or sleep.
