Betadine for nappy rash: what parents should know

Betadine contains povidone-iodine, an antiseptic intended to reduce microbes on skin. Most nappy rash is irritation from moisture, friction, urine, or stool, so an antiseptic does not correct the main cause and may further irritate delicate skin.

A typical irritant rash affects skin covered by the nappy and may spare deeper creases. A bright red rash involving the folds, sometimes with small spots beyond its edge, can fit a yeast infection and needs different care. Blisters, pus, crusting, open sores, or rapidly spreading redness can point to another problem that should be assessed.

Iodine can be absorbed through damaged skin, particularly when it is applied widely, repeatedly, or beneath an occlusive nappy. That is why applying Betadine without individual advice is unsuitable, especially for a newborn or a baby with thyroid or kidney concerns.

Change wet or soiled nappies promptly, rinse with lukewarm water or use fragrance-free wipes, and pat rather than rub. Allow short nappy-free periods and apply a thick, fragrance-free barrier ointment to shield skin from moisture and friction. Avoid talcum powder, perfumed products, harsh soap, and tight nappies.

If Betadine has already been applied, gently remove any excess and do not keep reapplying it unless a clinician gave precise instructions. A suspected yeast rash may require an age-appropriate antifungal; do not combine products without checking that they are suitable for the baby.

Arrange prompt assessment if the rash is worsening, unusually painful, spreading beyond the nappy area, or not improving with gentle barrier care. Seek urgent help if the baby is feverish, very sleepy, feeding poorly, has extensive blistering or peeling, or seems generally unwell. Newborns with a significant rash should be assessed early.

Ask whether the pattern looks irritant, fungal, bacterial, or related to eczema; which barrier product is appropriate; and whether any prescribed medicine belongs on broken skin or in the creases. Mention every product already used, how often nappies are changed, and whether feeding, stools, or behaviour have changed.
