Benefits and limits of an episiotomy

It is a surgical cut in the perineum, the tissue between the vaginal opening and anus. A maternity clinician may recommend it when the baby needs to be born promptly, when instruments are required, or when more room is needed for a particular manoeuvre.

The main potential benefit is controlled enlargement of the vaginal opening when delay could make birth more difficult or unsafe. It may assist placement of forceps or a ventouse and can help the clinician complete an urgent birth. Whether it offers an advantage depends on what is happening at that moment.

An episiotomy also creates a wound that requires stitches and healing. Possible effects include pain, bleeding, infection, wound opening, discomfort during sex, and extension into deeper tissue. It does not reliably prevent every serious tear or later pelvic-floor symptom, so the decision is individual rather than automatic.

During labour, the clinician should explain why the cut is being proposed, what it involves, and what alternatives are available if time permits. You can ask how urgent the situation is and whether assisted birth is planned. Your preferences can be recorded antenatally, although the recommendation may depend on events during birth.

Afterward, follow instructions for hygiene and pain relief, change pads regularly, and keep the area clean and dry. Gentle pelvic-floor contractions can support recovery when comfortable. Perineal repair uses dissolvable stitches; ask when to resume exercise, driving, tampon use, and sex based on healing and symptoms.

Contact your maternity team promptly if pain is increasing instead of settling, the wound opens, or you notice spreading redness, swelling, pus, unpleasant odour, fever, or difficulty passing urine. Heavy bleeding, faintness, severe weakness, or feeling acutely unwell needs urgent care.

Arrange follow-up for ongoing pain, painful sex, bowel-control problems, trouble controlling wind, or a sense of pelvic heaviness. These symptoms deserve assessment and may benefit from wound review, pelvic-floor care, or referral to a specialist service.

Ask in which circumstances your maternity unit recommends episiotomy, how consent is handled during an urgent birth, and what pain relief is used before cutting and stitching. You may also ask about perineal support during birth, wound care, and access to postnatal pelvic-floor assessment.
