A hollow can cast a shadow that looks like a dark circle, yet dark circles have several possible components. Lower-eyelid bags, fluid retention, loose skin, cheek volume loss, and natural bone structure change whether filler will blend the area or make puffiness more noticeable. Thin skin can also reveal a bluish or uneven appearance if gel is too superficial.

Previous under-eye filler may remain or shift, so the injector needs to know what was used, where it was placed, and whether swelling followed.

Seek an in-person examination from a licensed clinician who regularly treats the under-eye region. Discuss eye disease, dry eye, allergies, facial surgery, previous filler, medicines, bleeding tendency, and any history of prolonged facial swelling. Ask whether treating cheek support, pigmentation, or allergy would better match the cause.

Following treatment, use only the recommended skincare and observe both eyes in good light. Temporary swelling or bruising can occur, and morning puffiness may be more apparent. Do not massage the area unless instructed, because placement and aftercare vary.

Sudden blurred or lost vision, double vision, eye pain, a drooping eyelid, severe unusual headache, weakness, or speech difficulty after filler requires emergency care. Alert the treating clinic while arranging help, but do not delay emergency assessment. Escalating pain or skin that turns pale, grey, purple, mottled, cool, or blistered also needs immediate injector assessment because blood flow may be impaired.

For non-emergency review, report persistent puffiness, a blue-grey cast, increasing asymmetry, firm lumps, warmth, redness, drainage, or fever.

Ask what is causing the visible hollow or darkness, why this formulation and injection plane suit your anatomy, and what alternative would address bags or pigment. Request a clear plan for follow-up, urgent contact, and the use of hyaluronidase if the result or a complication calls for dissolution.
