Behçet’s disease and erythema nodosum: what to know

These inflammatory bumps are one possible skin feature of Behçet’s disease. Their appearance alone cannot confirm the diagnosis because other illnesses and medicines can cause similar nodules.

Behçet’s disease is an inflammatory condition that can affect several parts of the body. In a person with Behçet’s disease, erythema nodosum-like lesions reflect inflammation in the tissue beneath the skin. They often feel warm and sore and may look bruise-like as they settle.

A clinician considers the whole pattern, including recurring mouth or genital ulcers, eye symptoms, joint pain and other skin changes. Examination and, when needed, tests help distinguish these lesions from infection, a blood clot or another inflammatory condition.

Arrange a review if the nodules are new, returning or unusually painful. Note when they appeared and photograph changes in colour or size; also record ulcers, fever, eye irritation, joint symptoms and recent medicines. This timeline can help your clinician judge whether Behçet’s inflammation is active.

Resting and raising sore legs may ease discomfort. Avoid rubbing or trying to drain the nodules. Treatment is tailored to the severity and to other organs involved; it may include anti-inflammatory medicines, medicines that adjust immune activity or oral corticosteroids. Do not start, stop or alter a steroid dose without prescriber guidance.

Get urgent medical assessment for new blurred vision, eye pain, marked light sensitivity, sudden weakness, difficulty speaking, a severe unfamiliar headache, chest pain, breathlessness, coughing blood, or a painful swollen limb. Behçet’s disease can inflame the eyes, nervous system or blood vessels, and these symptoms need prompt assessment rather than home monitoring.

Seek same-day advice if a nodule becomes rapidly more red, hot or swollen, drains fluid, or occurs with fever or feeling significantly unwell, because infection may need to be excluded.

Ask whether the nodules match erythema nodosum, whether another cause needs checking, and which symptoms would change your treatment plan. If medication is proposed, ask what it aims to control, how it will be monitored and which side effects should prompt a call.
