Beginning of ringworm: early signs and next steps

The patch may be round, oval, or irregular rather than forming a perfect ring. On darker skin, the affected area may look brown, grey, purple, or darker than nearby skin instead of bright red. Ringworm can occur on many body areas and may slowly enlarge or develop nearby patches.

Ringworm is a superficial fungal infection, not a worm. It can pass through direct skin contact, infected animals, shared towels or clothing, and contaminated surfaces. Eczema, psoriasis, contact reactions, and other rashes can resemble it, so appearance alone may not settle the diagnosis.

Steroid cream used without an antifungal can suppress redness while allowing the fungus to spread, making the border less typical and the diagnosis harder to recognise.

Keep the area clean and dry, wash your hands after touching it, and avoid sharing towels, clothing, razors, or sports equipment. Launder items that contact the rash and check household pets for areas of hair loss or scaling.

A pharmacist or clinician can advise whether a topical antifungal suits a limited skin patch. Apply any recommended product to the full affected area and surrounding edge, and follow its instructions even if the skin begins to look better. Infection involving the scalp, beard, nails, or a wide area may need prescription oral treatment.

Arrange an assessment if the rash is on the scalp, face, beard, or nails; is widespread; keeps returning; or does not improve with correctly used antifungal care. Seek earlier advice if you are pregnant, have diabetes, have reduced immunity, or are treating a young child.

Prompt care is needed if the skin becomes very painful, hot, swollen, drains pus, or is accompanied by fever, because these changes may indicate a bacterial infection or another diagnosis.

Ask whether the pattern needs a skin scraping or another test, which antifungal formulation fits the body site, how long to continue it, and when you should expect visible improvement. Mention recent contact sports, shared facilities, affected family members, pets, and every cream already applied.
