Before and after treatment for nail lichen planus

Ridges, thinning, splitting or separation already present in the nail plate will not vanish immediately. Early diagnosis matters because ongoing inflammation around the nail-forming tissue can lead to scarring and lasting distortion.

The clearest sign of control is healthier nail emerging from the base, not a sudden change at the tip. Photographs should capture the nail surface, cuticle and surrounding skin from the same angle. Marking where abnormal nail begins can help show whether damage is moving outward as the nail grows.

Improvement may differ from nail to nail. A scar-like bridge between the skin and nail plate, marked thinning or loss of part of the nail can persist even after inflammation settles. Treatment therefore focuses on preserving remaining nail-producing tissue as well as appearance.

Keep nails comfortably short and file rough edges gently. Wear gloves for wet work and cleaning chemicals, moisturise the nails and surrounding skin, and avoid biting, picking or pushing back damaged cuticles. Artificial nails and harsh removers can hide progression or trigger additional irritation.

Use prescribed treatment exactly as directed and attend follow-up so the nail base can be reassessed. Mention medicines, pregnancy plans and other health conditions before corticosteroid treatment. Do not apply leftover steroid preparations around the nail because strength, site and duration need to match the clinical plan.

Seek dermatology review promptly for new nail thinning, splitting from the base, progressive lifting, scarring over the nail surface or loss of nail. Earlier treatment may offer a better opportunity to limit structural damage than waiting for several nails to change.

Painful swelling, warmth, pus or redness spreading from a nail fold may indicate infection and needs assessment. Also report mouth soreness, genital symptoms, hair loss, or a new itchy skin eruption because lichen planus can affect areas beyond the nails and may alter the examination or treatment plan.

Ask which nail structures appear inflamed, whether fungal testing or another diagnostic step is needed, and which changes may still grow out. Discuss how response will be documented, what adverse effects to watch for, and what would prompt a switch between topical, injected or oral treatment.
