Before and after a dermatofibroma scar

The final mark depends on the lesion's size and depth, its location, closure method, skin tension, pigment response, and individual healing. Removal can flatten the original bump, but it cannot make the skin look untouched.

A dermatofibroma often extends deeper than the visible surface suggests. Surgical excision therefore removes tissue around and beneath it, and the closure may be longer than the original bump. Early scars can be pink, darker, lighter, raised, firm, or itchy before they settle.

A reliable comparison separates healing from recurrence. Photograph the area at the same distance and angle, and note whether a firm focus is growing beneath or beside the scar. Images cannot confirm the diagnosis of a new lump; examination and, when appropriate, tissue analysis provide more information.

Follow the surgeon's instructions for dressings, washing, activity, and any stitch removal. Avoid picking crusts or stretching the wound. Once the surface has closed and the clinician agrees, scar care may include moisturising, silicone products, gentle massage, and sun protection to limit colour contrast.

If removal is being considered, ask to see how the planned incision will sit relative to natural skin lines. Surgical excision offers tissue for confirmation, but the benefits must be weighed against bleeding, infection, wound separation, altered sensation, pigment change, and a thick or widened scar.

Contact the treating team for increasing redness, warmth, swelling, pain, pus, unpleasant odour, wound opening, or bleeding that does not settle with firm pressure. Fever or rapidly spreading redness needs prompt medical attention. Later, arrange review if a new or enlarging lump appears in or near the scar, or if the scar becomes persistently painful or restricts movement.

Ask how confident the clinical diagnosis is, whether removal is necessary, how large the expected scar may be, and how the wound will be closed. Clarify whether the specimen will be examined, which activities could pull on the closure, and what options exist if the scar becomes raised, dark, itchy, or wide.
