Before and after ashy dermatosis: what to expect

Ashy dermatosis, also called erythema dyschromicum perstans, causes grey-brown or blue-grey patches. Improvement may be subtle, and lighting, camera settings, moisturiser, and tanning can make photographs look different even when the skin has changed little.

Useful photographs are taken in the same room, at the same distance, with similar lighting and no filter. Compare the same patch for fading, enlargement, new areas, or a change at the edge. Texture should also be noted: ashy dermatosis is primarily a colour change, so scale, crusting, open skin, or marked swelling may suggest another process that deserves examination.

Pigment can fade unevenly. A patch may look mottled while colour is changing, and the contrast may shift after sun exposure. A clinician can distinguish active disease from leftover pigmentation and from other causes of dark patches.

Take periodic photographs under consistent conditions and note whether new patches appear. Use a gentle cleanser, moisturise if the skin feels dry, and protect exposed areas from strong sunlight to reduce extra contrast. Avoid scrubbing, bleaching mixtures, and unprescribed steroid creams, which can irritate or damage skin.

Treatment is individual because no single option works predictably for everyone. A dermatologist may discuss anti-inflammatory topical medicines, including topical calcineurin inhibitors, after confirming the diagnosis and considering the site and skin response.

Book a dermatology assessment if patches are spreading, the diagnosis is uncertain, or treatment causes burning, thinning, or a new rash. Seek prompt care for blistering, skin breakdown, significant pain, fever, facial swelling, or trouble breathing. These features are not explained by a simple pigment comparison and need direct assessment.

Ask what features support ashy dermatosis, whether a skin sample is needed to exclude a similar-looking condition, and which changes would show that the process is active. Clarify how to apply any medicine, what irritation to watch for, and when photographs should be reviewed.
