Check whether swelling began suddenly, followed a fall or twist, or developed with repeated hand use. Bruising, warmth, redness, stiffness, a lump, numbness, and reduced grip provide useful clues, but appearance alone cannot identify the cause.
What the swelling may reflect
After trauma, swelling can accompany a sprain, tendon injury, joint injury, or fracture. Gradual swelling with pain during thumb or wrist movement can reflect irritation around a tendon or changes within the joint. De Quervain's tenosynovitis often presents with pain and swelling on the thumb side of the wrist, though these features are not specific to it.
Hot, red, increasingly tender skin raises a different concern, particularly with a cut, bite, puncture, or fever. A clinician may examine circulation, sensation, tendon function, joint stability, and the precise area of tenderness before deciding whether imaging or another test is needed.
What to do initially
Remove rings or tight jewellery from the affected hand before swelling increases. Rest the thumb from heavy gripping and painful pinching, elevate the hand, and apply a cool pack wrapped in cloth for short periods. Avoid placing ice directly on the skin.
Gently move the thumb within a comfortable range unless an injury may have caused a fracture or dislocation. Do not force the joint straight or attempt to correct a changed shape. A pharmacist can help assess whether an over-the-counter pain medicine is compatible with your health conditions and current medicines.
When to seek care
Seek urgent assessment if the thumb is misshapen, blue or unusually pale, numb, extremely painful, or impossible to move after an injury. An open wound with visible bone, rapidly expanding swelling, or loss of normal hand feeling also needs emergency care.
Obtain prompt medical advice for spreading redness, marked warmth, drainage, fever, or worsening swelling after a bite or wound. Arrange a routine appointment when unexplained swelling persists, repeatedly returns, or restricts normal grip and daily activities.

Can tendon irritation cause swelling near the thumb base?
Yes. Irritation around the thumb-side tendons can present with local pain and swelling, especially during gripping or thumb and wrist movement. Other problems can look similar.
Should I use heat on a newly swollen thumb?
A wrapped cool pack is generally more suitable immediately after injury or while swelling is active. Avoid strong heat when the area is newly injured, hot, or visibly swollen.
