Why does a Bartholin gland cyst keep coming back?

The gland continues to make lubricating fluid. If the duct remains narrowed, or a drainage incision heals shut before a stable passage forms, fluid can build up in the same area. Recurrence does not necessarily mean that anything you did caused the cyst.

A swelling in the same place may be another uncomplicated cyst or an abscess if bacteria infect the trapped fluid. An abscess is more likely to present with pronounced tenderness, heat, redness, throbbing, pus, or fever. Examination matters because boils, skin cysts, and other vulval lumps can occur nearby and may need different care.

Tell the clinician how often the lump has appeared, whether it fully disappeared between episodes, how previous episodes were treated, and whether drainage brought relief. This pattern helps distinguish incomplete healing from a fresh blockage.

Comfortably warm baths can reduce mild discomfort while you arrange care. Avoid squeezing the lump, using sharp objects, or applying harsh products. A returning cyst should be assessed rather than repeatedly managed by forceful home drainage.

A clinician may drain the cyst and leave a small catheter in place temporarily, allowing tissue to heal around a new passage. Marsupialisation creates a wider drainage opening and may be considered when cysts return. The appropriate approach depends on current infection, previous procedures, the examination, and your preferences. Antibiotics are reserved for situations in which infection findings make them useful.

Get prompt assessment if the recurrent swelling becomes rapidly larger, intensely painful, red or hot, releases pus, or occurs with fever or feeling unwell. Seek planned evaluation for a lump that never fully clears, feels hard or fixed, appears in a different location, or develops after menopause. These findings call for confirmation that the swelling is truly arising from the Bartholin gland.

Ask whether the earlier drainage opening has sealed, whether the current lump is infected, and which procedure is suited to repeated episodes. Clarify how long any catheter should remain, how to care for the site, when normal activity can resume, and what the next option would be if another cyst forms.
