What does a black dot on a suspected Bartholin cyst mean?

The spot could reflect a blocked skin pore, an ingrown hair, dried blood, a tiny scab, or pigmentation over a different kind of lump. Its colour cannot establish what lies beneath the skin.

A Bartholin cyst is a fluid-filled swelling caused by blockage of a duct near the lower vaginal opening. It is usually felt beneath the skin and does not require a visible central pore. A distinct dark opening can instead point toward a skin-based lesion, although conditions can overlap.

Location is useful: a Bartholin swelling sits beside the vaginal entrance, whereas a hair-follicle bump may occur on hair-bearing outer skin. Only an examination can reliably assess the lump's depth, attachment, colour, tenderness, and drainage.

Do not pick the dot, squeeze the lump, or try to open it with a needle. These actions can cause bleeding, scarring, and infection. Wash with water or a gentle unperfumed cleanser, pat dry, and avoid shaving directly over the area.

A warm compress can ease mild soreness. Note whether the spot is new, changing in colour or shape, bleeding, crusting, or accompanied by discharge. Arrange a clinical review for a persistent, growing, recurrent, or uncertain vulval lump.

Seek prompt help if pain becomes severe, the swelling expands rapidly, redness spreads, pus appears, or you develop fever, chills, or feel unwell. Ongoing bleeding, an open sore, or a dark area that is enlarging or changing also deserves timely examination.

Any new vulval lump after menopause should be assessed even if it causes no pain. This precaution allows a clinician to distinguish a blocked Bartholin duct from skin conditions and other less common causes.

Ask whether the lump arises from the Bartholin gland or the skin, what the dark spot represents, and whether infection or bleeding is present. You can also ask if treatment should involve observation, medication, drainage, or investigation of the surface change.
