Bartholin cyst swelling: what it can mean and what to do

A small cyst may cause no discomfort, while a larger or infected cyst can become tender and interfere with sitting, walking, or sex. A clinician can distinguish a simple cyst from an abscess and discuss whether observation, drainage, or another treatment fits your symptoms.

The Bartholin glands sit near the lower part of the vaginal entrance and release lubricating fluid through narrow ducts. If a duct is blocked, retained fluid can form a soft lump, often on one side. Swelling alone does not show whether infection is present.

Increasing pain, warmth, redness, pus, an unpleasant discharge, fever, or feeling unwell can accompany an abscess. Other vulval lumps can look similar, so a new or persistent swelling deserves an examination rather than squeezing or trying to identify it at home.

Use a warm bath or warm compress for comfort, keep the area clean and dry, and wear loose clothing that does not rub. Avoid puncturing, squeezing, or inserting anything into the lump because this can injure tissue and spread infection.

Arrange a routine appointment for a new lump, a swelling that persists, or repeated episodes. Treatment depends on pain, size, infection, and recurrence. If drainage is needed, a clinician may discuss a small catheter or marsupialisation, which creates an opening that allows fluid to drain.

Seek urgent medical assessment if the swelling rapidly enlarges, becomes severely painful, hot, or very tender, drains pus, or occurs with fever or feeling generally unwell. Get prompt help as well if pain makes walking, sitting, passing urine, or managing normal activities difficult.

Do not wait for a routine visit if redness is spreading, you feel faint or confused, or your condition is worsening quickly. A clinician can check for an abscess and drain it safely when appropriate.

Ask whether the swelling appears to be a cyst or abscess, whether testing is needed, and what signs should bring you back urgently. If it has recurred, ask how a catheter and marsupialisation differ, what aftercare involves, and how discomfort should be managed.
