When a Bartholin cyst is painful to touch

A blocked duct traps lubricating fluid near the vaginal opening and forms a cyst. Pressure from a growing cyst can cause tenderness. When infection produces an abscess, the lump often becomes more painful, swollen, warm or red and may interfere with sitting, walking or sex. Tenderness alone cannot confirm infection.

Notice whether pain occurs only with direct pressure or continues at rest. A rapid increase in pain or swelling, hot skin, pus-like or unpleasant-smelling discharge, fever and feeling unwell support concern about an abscess. A clinician can also check for other causes of a painful vulval lump, because not every swelling beside the vaginal opening comes from a Bartholin gland.

Do not repeatedly press the area to test it. Frequent handling adds friction and may worsen soreness without providing a reliable answer.

Soak the area in comfortably warm water and wear loose, breathable clothing that does not rub the swelling. Pause cycling, penetrative sex or other activity that directly increases pressure. Keep the vulva clean with water or a gentle unperfumed cleanser, then pat rather than rub it dry.

Obtain urgent assessment when pain is severe, escalates quickly, prevents normal movement or sitting, or occurs with fever, spreading redness, marked swelling or feeling significantly unwell. An abscess may need a clinician to drain it; antibiotics are used when the clinical picture indicates they are needed.

Arrange a non-emergency visit if mild tenderness does not settle with warm-water care, the lump enlarges, or it returns after improving. Any new vulval lump should be examined, with particular promptness after age 40 or after menopause.

Ask whether the examination suggests a cyst, an abscess or another type of lump, and whether testing the drainage is useful. If a procedure is proposed, ask how it keeps the duct open, what wound care is required, which activities to avoid during healing, and what recurrence would look like.
