Texture alone cannot confirm whether the swelling is a simple blocked gland, an abscess, scar tissue, or another type of vulval lump.

Why the lump may feel hard

A blocked Bartholin duct can trap fluid near one side of the vaginal opening. Pressure inside the cyst may make its surface feel tight or firm. Inflammation can also thicken the surrounding tissue, while an abscess is often increasingly tender, warm, red, and swollen.

A clinician assesses the exact position, mobility, tenderness, skin changes, and whether the lump is solid or fluid-filled. Firmness that remains after pain settles deserves examination rather than repeated squeezing.

Safe next steps

Arrange a routine appointment if this is a new hard lump, if its location is uncertain, or if it does not soften or shrink. Until then, a warm shallow bath or warm compress may ease discomfort. Keep the area clean and dry, wear loose breathable underwear, and avoid friction when it hurts.

Do not pierce, cut, or forcefully drain the swelling. If drainage is needed, a clinician can discuss methods that keep an opening available for continued drainage, including marsupialisation in selected cases.

Seek same-day medical care for rapidly worsening pain or swelling, fever, feeling generally unwell, spreading redness, pus, or difficulty sitting, walking, or passing urine. These changes can accompany infection and may require drainage or medicine.

Book an examination promptly if the lump feels fixed, has an irregular outline, causes broken or bleeding skin, keeps enlarging, or persists after other symptoms improve.

Not necessarily. Internal pressure or surrounding inflammation can cause firmness. Increasing tenderness, warmth, redness, pus, fever, or feeling unwell makes infection more concerning.

Can I drain a firm Bartholin cyst myself?

No. Cutting or piercing it can injure tissue and introduce infection. Warm water may support natural drainage, but painful or persistent swelling should be assessed.
