What if a Bartholin cyst contains blood?

A small blood streak can follow friction, squeezing, spontaneous drainage or injury to delicate surface tissue. However, appearance alone cannot show whether the blood came from the cyst, nearby skin, the vagina or the urinary tract.

An uncomplicated Bartholin cyst contains gland fluid, while an abscess contains infected material. Inflamed tissue can bleed easily, especially if the lump has rubbed against clothing, been manipulated or opened. Blood mixed with pus-like drainage may accompany an irritated or infected collection.

Not every bleeding lump beside the vaginal opening is a Bartholin cyst. A skin injury, another type of cyst, a blood-filled swelling or a different vulval condition may look similar. Direct examination is the reliable way to identify the source and decide whether sampling or drainage is appropriate.

Do not squeeze, lance or repeatedly inspect inside the opening. Apply a clean pad rather than inserting material into the lump, wash the outer area gently with water and note the colour, amount and source of any further fluid. Avoid sex and friction if these trigger pain or bleeding.

Contact a clinician for an examination, particularly if the bleeding recurs or the diagnosis has never been confirmed. Mention recent injury, pregnancy possibility, medicines that affect clotting, menstrual timing, discharge and urinary symptoms because these details help locate the source.

Seek urgent care if bleeding is heavy or does not stop, you feel faint or weak, pain or swelling escalates quickly, fever appears, redness spreads, or you cannot pass urine comfortably. Use emergency services for severe bleeding or collapse.

Prompt review is also appropriate for bleeding after menopause, a hard or irregular lump, an ulcer, altered skin colour, or a swelling that persists after drainage. These signs do not establish a cause, but they should not be managed as a routine cyst without examination.
