Blood after a Bartholin cyst bursts

The cyst wall and nearby vulval skin contain small blood vessels, so drainage can look pink, red, or brown after the area opens. Pus, cloudy fluid, or an unpleasant smell may indicate that the cyst was infected. Bleeding should lessen rather than continue soaking dressings.

Relief of pressure and pain after fluid escapes is common, but an opening at the surface does not show whether the cavity has emptied fully. The duct can remain blocked, the opening can close too soon, or infection can persist underneath. Continuing tenderness, a lump that refills, increasing warmth, or thick discharge merits examination.

Blood may also come from irritated skin, shaving injury, menstruation, or another vaginal or vulval source. If you cannot tell where it originates, use a clean pad to observe the pattern and arrange an assessment rather than inserting anything into the opening.

Wash your hands, rinse the outside gently with warm water, and pat dry. Use a clean, unscented pad or gauze to absorb drainage and change it whenever damp. Warm sitz baths can support comfort and continued drainage. Avoid fragranced products, tight clothing, sex, tampons, squeezing, and attempts to widen the opening while it heals.

Contact a clinician if the lump remains, the discharge continues, or this has happened before. They can check for an abscess and decide whether medication or a drainage procedure is needed. Marsupialisation may be discussed when cysts or abscesses recur.

Seek urgent medical care if bleeding is flowing, does not slow with gentle external pressure, repeatedly soaks pads, contains large clots, or is accompanied by dizziness, faintness, weakness, or severe pain. Do not pack the opening or apply antiseptic inside it.

Prompt assessment is also needed for fever, chills, spreading redness, rapidly returning swelling, worsening pain, foul-smelling pus, or feeling very unwell. These signs are driven by possible infection or blood loss, regardless of how commonly a cyst may rupture.

Tell the clinician when drainage began, what the fluid looked and smelled like, whether bleeding is slowing, and whether pain or fever is present. Ask if the cavity is still blocked, whether infection treatment is appropriate, how long to avoid friction, and what plan could reduce repeated episodes.
