Managing a Bartholin cyst during menstruation

Period blood does not need to be cleaned from inside the vagina. Focus on changing menstrual products regularly, washing the outer vulval area gently and watching the lump for signs of infection.

A Bartholin cyst is trapped fluid near the vaginal opening caused by a blocked gland duct. Menstruation does not create the blockage, although pads, tight clothing, moisture or repeated wiping may make an existing tender area feel more irritated.

Some people notice vulval sensitivity around their period and may become more aware of the lump then. Tracking whether it shrinks after bleeding ends can help describe the pattern, but a persistent or enlarging lump still deserves clinical assessment.

Choose an unscented menstrual product that does not press or rub against the swelling. Change it before it becomes damp, wash your hands before and after, and rinse the vulva with water rather than fragranced washes or douches.

A warm sitz bath can soothe soreness. Pat dry, wear breathable underwear and avoid squeezing the cyst. If inserting a tampon or menstrual cup is painful, stop and use an external product that feels comfortable until the area has been assessed or settles.

Arrange prompt care if the lump rapidly enlarges, becomes intensely painful, feels hot or develops surrounding redness. Fever, chills, pus, feeling generally unwell, or pain that makes sitting or walking difficult can indicate an abscess.

Get urgent help for faintness, confusion, severe weakness or rapidly spreading redness. Also seek an examination for a new vulval lump after menopause, even when it is not painful.

You might ask whether the lump is a cyst or abscess, whether any menstrual product could aggravate your symptoms, and whether drainage is needed. If episodes recur around bleeding, ask what details to record and whether another cause of cyclical vulval pain should be considered.
