Bad poison ivy rash: what to do

After a likely exposure, wash the skin and contaminated belongings to remove remaining plant oil. Once the oil is gone, fluid from rash blisters does not spread the reaction to another person or to a new body area.

Poison ivy oil triggers contact dermatitis in sensitised skin. Red, intensely itchy patches, bumps, or blisters may follow lines where leaves brushed the body. Different areas can erupt at different times because exposure and skin absorption vary, not because the rash is travelling internally.

Oil can remain on clothing, footwear, gloves, tools, pet fur, and beneath fingernails until cleaned. Recontact with those surfaces can create fresh sites. Scratching may damage skin and allow bacterial infection even though it does not redistribute clean blister fluid.

Rinse exposed skin promptly with plenty of lukewarm water and soap, cleaning under the nails without harsh scrubbing. Wash clothes separately and clean washable gear while wearing protective gloves. Avoid burning the plant because airborne particles can affect the face and airways.

Cool wet compresses, a cool bath, loose clothing, and fragrance-free moisturiser can reduce irritation. Keep nails short and cover weeping areas with a clean non-stick dressing. A pharmacist or clinician can advise on an appropriate anti-itch product or corticosteroid; extensive involvement may require prescription treatment.

Seek emergency help for trouble breathing, throat tightness, faintness, or swelling around the eyes, lips, tongue, or face. Smoke exposure followed by coughing or breathing symptoms also needs urgent assessment.

Contact a clinician promptly when the rash is widespread, severely swollen, involves the eyes, mouth, genitals, or large skin folds, disrupts sleep despite self-care, or continues to worsen. Increasing pain, warmth, pus, fever, or expanding redness can indicate secondary infection.

Ask whether the distribution fits plant contact or another rash, whether the eyes or airways require separate assessment, and which medicine strength is appropriate for each body site. Explain when exposure occurred, what was cleaned, and which treatments have already touched the skin.
