What should you know about bad nappy rash broken skin?

Short answer
Broken skin from nappy rash needs gentle cleaning, frequent nappy changes, a protective barrier and timely clinical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Cleaning
- Rinse gently and pat dry without rubbing.
- Protection
- Use a thick fragrance-free barrier after changes.
- Avoid
- Powders, antiseptics and fragranced products on broken skin.
Why the skin can become raw
Prolonged contact with urine or stool, friction and repeated wiping can damage the skin barrier. The area may look bright red, weep, crack or bleed slightly during cleaning. Once the surface is open, urine and stool sting more and bacteria or yeast can take advantage of the damaged barrier.
A rash that reaches the skin folds with small red spots nearby can occur with yeast, while honey-coloured crusts, pus or rapidly spreading redness raise concern for bacterial infection. Appearance alone may not identify the cause, especially when several irritants are involved.
How to care for the area
Change wet or soiled nappies promptly. Rinse with lukewarm water or use a very soft damp cloth, then pat dry rather than rubbing. Allow brief nappy-free time on a washable surface. Apply a thick, fragrance-free barrier layer so moisture contacts the product instead of the injured skin; remove only visible soiling rather than scrubbing every trace away.
Avoid fragranced wipes, talcum powder, antiseptics and unprescribed steroid or antifungal combinations on open skin. A clinician can decide whether an emollient, antifungal medicine or another treatment matches the cause.
When your baby needs medical care
Seek prompt medical advice when skin is broken, the baby seems very uncomfortable, or the rash is not beginning to settle with careful barrier care. Urgent assessment is needed for fever, unusual sleepiness, poor feeding, spreading redness, swelling, blisters, pus, extensive peeling or bleeding that continues. A young infant who appears unwell should be assessed without delay.
Useful questions at the visit
Ask whether the pattern suggests irritation, yeast or bacteria, which cleanser and barrier are suitable for open skin, and whether medicine should go beneath or above the barrier. Confirm what improvement should look like and which changes require another examination.

Questions people ask
Should all the barrier cream be wiped off at every change?
No. Remove stool and the soiled surface layer gently, then replenish the barrier. Repeated scrubbing can further injure raw skin.
Can antifungal cream be used on broken nappy skin?
Only use a product that a clinician or pharmacist confirms is appropriate for the baby's age, the suspected cause and the condition of the skin.
